Hot flashes and night sweats are some of the most common (and disruptive) symptoms of perimenopause and menopause—but they’re also some of the most misunderstood.

Let’s talk about why they happen—and how acupuncture and Chinese medicine can help.


What Causes Hot Flashes and Night Sweats?

From a Western Medicine Perspective


Hot flashes are primarily driven by hormonal fluctuations, especially declining estrogen.

Estrogen plays a key role in regulating the hypothalamus—the part of your brain that controls body temperature. When estrogen drops:


  • Your “internal thermostat” becomes more sensitive

  • Small temperature changes trigger a heat response

  • Blood vessels dilate suddenly → you feel flushed

  • Your heart rate increases → palpitations

  • Your body tries to cool down → sweating


At night, this process is even more noticeable because:


  • Cortisol naturally shifts in the early morning hours

  • Blood sugar can dip

  • Body temperature cycles change during sleep


This is why many women wake up around the same time each night, often between 1–4 AM, just before a hot flash hits.


From a Traditional Chinese Medicine (TCM) Perspective


In Chinese medicine, hot flashes and night sweats are not just “hormones”—they’re a pattern.

Most commonly, we see:


  • Kidney Yin Deficiency

  • Sometimes combined with Heart Yin deficiency or Liver Qi stagnation


What does that mean in simple terms?


  • Yin represents cooling, nourishing, and grounding energy

  • When Yin is depleted, the body has less ability to anchor heat

  • Heat rises → causing flushing, irritability, insomnia, and night sweats


That “waking right before the heat hits” is something we see all the time clinically. In TCM, it reflects:


  • Yin not anchoring Yang at night

  • Often paired with Heart involvement → palpitations, anxiety, vivid dreams


Why Symptoms Happen During the Day and Night


Many women notice:


  • Random daytime hot flashes

  • But more intense night sweats


Here’s why:


Daytime

  • Stress, caffeine, alcohol, and blood sugar swings can trigger episodes

  • Liver Qi stagnation (stress pattern) can push heat upward


Nighttime


  • Yin should be strongest at night (cooling, restorative)

  • When it’s deficient, heat becomes more pronounced

  • The body is still → making symptoms feel stronger

  • Hormonal rhythms peak in early morning hours


How Acupuncture Helps Hot Flashes & Night Sweats


Acupuncture works by regulating the nervous system, hormones, and circulation.

Clinically, we often see:


  • Reduced frequency of hot flashes

  • Less intensity when they do occur

  • Improved sleep

  • Fewer nighttime awakenings

  • Decreased palpitations and anxiety


There is also research supporting its benefits.

For example, the ACUFLASH randomized controlled trial found that women receiving acupuncture had a significant reduction in hot flash frequency and intensity compared to controls, along with improved quality of life.


A broader Cochrane review on acupuncture for menopausal hot flushes found that while results vary, acupuncture may reduce the severity of hot flashes and improve overall menopausal symptoms, especially sleep and well-being.
